To Our Patients and Community,
After many wonderful years of serving this community, Dr. Martin Family Practice will be permanently closing its doors this summer. We want to thank you for the trust you have placed in us for your healthcare needs.
Key Closure Dates:
- Last Day for In-Office Appointments: July 31, 2026
- Last Day for Televisits: August 8, 2026
After August 8, 2026, the Practice will be fully closed and no longer able to provide medical services, prescription refills, or referrals.
Continuity of Care:
Your health remains our priority. We strongly advise you to establish care with a new physician promptly to ensure there is no gap in your medical treatment. You may contact your health insurance provider for a list of in-network physicians in your area.
Medical Records Information:
In accordance with Florida state law (Fla. Stat. ยง456.057), your medical records are strictly confidential and will be securely maintained for a minimum of five years from your last patient contact.
How to Obtain your Records:
We have appointed a secure records custodian to manage all medical record requests and transfers. To request a copy of your records or have them sent to your new doctor, please contact:
